Automating Patient Form Follow-Ups for a Mental Health Provider

OfficePro Consulting built a follow-up automation layer for a mental health provider on top of its Microsoft Fabric data warehouse. Scheduled flows check daily for patients with outstanding forms, send personalised reminders, escalate the ones that go unanswered and close off automatically once a form is returned.

The challenge: chasing outstanding forms by hand

Every new patient is sent intake forms and further forms go out at set points across a course of treatment. Completion mattered, but keeping track of who had and had not responded was a manual job that pulled clinical staff away from care.

What we built

Two scheduled Power Automate cloud flows that run against the provider's Microsoft Fabric data warehouse and drive the whole follow-up cycle without staff intervention.

How it works

Each run pulls the current list of outstanding forms from Fabric. For each patient the flow checks where things stand and takes the right step: send a first reminder, send a follow-up where one is due, escalate to a nominated staff member once a patient has been reminded a set number of times, or close the record once the form is returned. Reminders are branded emails sent from a central shared mailbox, each linking the patient straight to the correct form. The behaviour is controlled through configuration rather than code, so the provider can adjust timing, escalation and content without changing the flow.

The results: an automated follow-up loop, live in production

What changed once the flows went live, from manual chasing to a loop that runs itself.

Daily
Runs every morning on its own
Manual chasing→Scheduled flow
Auto-close
Closes the moment a form is returned
Manual check→Every cycle
One mailbox
Consistent reminders across every patient
Individual staff→Shared mailbox
Config-led
Provider adjusts timing and content
Needs a developer→No code change
The follow-up process that used to sit with clinical staff now runs on its own each day. Patients with an outstanding form are identified, reminded, escalated where they stay unanswered and closed off automatically once they respond, so staff are no longer tracking completion or sending chasers by hand. Because reminders send from one shared mailbox and link straight to the right form, follow-up stays consistent across the whole patient base rather than depending on individual staff remembering to act. And because the timing, escalation and content are controlled through configuration, the provider can adjust the system without coming back to a developer.
